Requirements for New Facility Registration

- The type of facility to be registered,
- Services to be rendered,
- Name of facility,
- The exact location and address where the intended business is to be undertaken.
- Certificate of full registration with CAC
- Certificate of annual practicing licence
- Degree certificate and other relevant credentials as applicable to the staff
- Newspaper change of name for married females
- 2 passport photographs of the proprietor, all of which shall be endorsed by one of the applicant’s referees, shall be attached to the application. This means one of your referees will sign behind your passport.
- 2 passport photographs of the health practitioner in charge and other employees
- Tax clearance certificate (3 years tax clearance prior to the current year or tax identification number certificate)
- Building layout of the facility (4copies)
- Building plan of the facility (4 copies)
- List of equipment
- Direction to the facility (4 copies)
- A letter of good standing with the professional body relevant to the health facility
- A letter of commitment to share patients attendance, immunization, morbidity, mortality statistics and any other relevant data with the Ministry of Health on a monthly basis or any other stipulated time
